• Green Barcoded ID or Smart ID Card: Proof of identity
• Proof of Residence: Not older than 3 months
• Latest Bank Statement: Last 3 months
• SASSA Card or Reference Number
• Medical Records: For disabled dependents
•Letter from Tribal Authority: For rural applicants
